Do you accept my insurance?

Keen Eyecare works with all types of insurances. We currently bill with:

If you do not see your carrier, please call our office.

What do I need to bring to my appointment?

For identity and insurance purposes, please bring:

  • driver's license (or other  documented proof of identity)

  • Vision and medical insurance cards 

  • Name of policy holder's name, date of birth, last four to social security #, place of employment 

To speed up the check-in process, please print and bring your filled out paperwork to your exam. You can print the PDF below:

- HSA/FSA -

We happily accept both Flexible Spending Accounts (FSA) and Health Savings Accounts (HSA) to cover the cost of your eye exams, contacts, and other vision expenses. The only difference is that FSA funds expire while HSA funds do not.
